“I think you forgot something…”

Over a decade ago I was working as a paramedic. RSI’d a patient with respiratory failure. Transferred to ED team no problem. An hour or so later I come in with another patient and the nurse says “that patient that you brought in intubated, I think you forgot something…” and showed me this X-ray. It was terrifying for a minute till I realized the disposable blade simple rolled under her 🤦‍♂️😂
