Point me in the right direction?

Hi! I had this x-ray done of my foot last week. The doctor told me the appropriate treatment is “exercise.”

I injured it about a year ago - I thought I felt it break but the doctor at that time told me it was a sprain.

Just looking for some guidance or opinions as I continue to seek further medical advice (from a new doctor). I don’t think my current doctors are giving me the best diagnoses. Thanks!
