More detailed notes

I work at a university where the ordering providers rarely put notes as to why this patient is getting an X-ray. I have to transcribe their order into Epic for a Rad read where a reason is required. I include when it happened, attention to what area, and an prior history to that area. Example- Rt Ankle Injury x 3 days, attn. lateral aspect, h/o sprains.
What else can I add that would be useful?
