Why isn't everything seen on imaging reported on report?

So my question is, why isn't everything noted on a radiology report? For example, on two different abdominal MRI's one reports large, small bowel, and lymph nodes but the other one does not. Or not reporting of spine on abdominal mri.

Does this depend on the radiologist and can things be missed this way?
