I posted this somewhere else but thought maybe someone here could help.
25 M, 5’11, 200 lbs
Back in early June I went to my PCP after I found a little grain of rice sized bump on my left testicle. He felt it and didn’t think it was anything serious but sent me for an ultrasound just to be sure.
The ultrasound came back negative and these were the results:
History: Mass.
Scrotal ultrasound:
Multiple images from a scrotal ultrasound exam are reviewed.
The right testicle measures 3.1 x 4.8 x 2.2 cm. Shows homogenous echogenicity with no focal intratesticular lesion. Epididymis is unremarkable. Color and Doppler imaging shows normal-appearing vascularity.
There is a small right-sided hydrocele noted.
The left testicle measures 3.6 x 4.8 x 2.2 cm. Demonstrates homogenous echogenicity with no intratesticular lesion. Epididymis appears to be normal.
Color and spectral Doppler imaging shows normal-appearing vascularity in the left testicle. Small left-sided hydrocele is noted.
No hernias are identified. No inguinal abnormalities are seen.
No scrotal wall thickening or varicoceles identified.
IMPRESSION Impression: 1. Small bilateral hydroceles. 2. No other significant abnormalities are identified.
He said everything sounded okay and he would refer me to a urologist for a second opinion. I went to my appointment and saw an NP at the urology office. She felt the lump and said she didn’t think that was the hydrocele that was seen on my ultrasound. She thought the lump was missed on the first ultrasound and ordered me another ultrasound for that day. The tech who did that ultrasound was very thorough and had me point out the lump to her so she could go over that area. That ultrasound said the same thing except it also picked up a 5mm Epididymal cyst on my left testicle. (Which I assumed was the lump I was feeling the whole time)
Today I told my PCP the results of that ultrasound and he said that he referred me for another Urology appointment on Monday. I asked why I would need to do that after two ultrasounds and he said “ultrasounds are user dependent, you would benefit from another appointment ” so it seems to me that he doesn’t trust either ultrasound.
I thought that ultrasounds were very accurate and reliable, is there any reason a PCP wouldnt trust them? What are the odds that two physical exams and two ultrasounds would have missed a tumor? Has anyone here seen a similar situation?
