46 yo female with acute pyelonephritis

A 46 year old presented with acute pyelonephritis. CT scan showed a large left sided hydroureter. Renal nuclear scan showed a nonfunctional left kidney.

First images are the CT showing the obstruction. The following are the cystogram and retros with a clear obstruction point.

She underwent left nephrectomy with complete removal of the left ureter.

Final path - obstruction from endometriosis.

The patient had no menstrual symptoms and silently killer her kidney.
